Review Comments

My review will not appear at first glance to reflect my belief that I would really like the authors to take my suggestions to heart, revise, and then publish this.  I love the incorporation of opportunity costs.  To that end please take my extensive comments on both the form and the documents as those of a colleague who wants to help you.  I was committed to your work and so spent an afternoon working to provide you with actionable feedback in ways I hope you can understand.  As for issues a brief overview. First, it is concerning that the authors communicate prior comments they received but then do not incorporate the changes into the CI and TN.  This would typically be an automatic rejection to other journals but I think a deferral is more appropriate.  It may just be an oversight in understanding the value and importance of comments and responding to them. There are significant issues that lead to my conclusion this should be deferred.  In the future, any comment from discussants or reviewers should be put into a table with column 1 recounting the comments and column 2 detailing exactly how the comments were addressed, where this occurs in the manuscript or a detailed explanation of why comments were not incorporated.  This is a work in progress and I hope to see it published in the future.
